Before you visit our office or schedule a specific test or procedure, it is important to know that your insurance plan may require multiple co-pays, deductibles, and coinsurance, as well referrals or pre-authorizations. Coverage will depend on the type of plan you have chosen.

What should I ask before an office visit?

Since procedures and visits are varied in preparation instructions, our staff will call you 1-2 days in advance of your appointment and inform you â€‹of everything you need to know before your visit.

What is co-payment, deductible, and co-insurance?

A co-payment is an amount determined by your specific insurance that you are responsible for each time you get medical services. It typically ranges from $5-$50 per visit, depending on your specific insurance provider. A deductible is an annual amount charged by your insurance provider as a bulk sum. Co-insurance is

Do you accept Medicare and Medicaid?

We accept Medicare. We also accept Medicaid as a secondary insurance ONLY (after Medicare).

What if I need authorization or referral before my visit?

We will acquire authorizations for any procedures performed at our offices. You will need to get a referral from your primary physician before your visit.

What if I have no medical insurance?

We accept cash, check and credit card payments for all services. Depending on your specific situation, we are able to provide flexible pricing.

What if my insurance changes during a course of treatment?

Make sure to contact our office and provide your new insurance information to ensure coverage.
